I’ll tell you something I don’t share at dinner parties: I struggled with pelvic pain from the time my husband and I got married all the way until the birth of our first son 3 ½ years later. So when you sit across from me and tell me intimacy hurts, you’re not talking to a clinician who has only read about it in a textbook — you’re talking to someone who has lived it.
Here’s what’s usually going on: the pelvic floor muscles are held in a state of guarding — too tight, too protective, unable to lengthen and relax. Like a perpetually clenched jaw, except nobody can see it and everybody’s too embarrassed to mention it. Pain leads to more guarding, guarding leads to more pain, and the cycle digs in. Why does sex hurt?
Usually because the pelvic floor muscles are too tight and guarded to lengthen and relax. Scar tissue, hormonal changes, and a protective nervous system can all play a role — an evaluation tells us exactly which ones apply to you.
Is this common after having a baby?
Very. Pelvic pain and painful intimacy are common after childbirth and during hormonal shifts like menopause, but common doesn’t mean permanent. This is highly treatable.
